A North Carolina-based forest products company has been cited by the Occupational Safety and Health Administration following the death of a worker earlier this year at its Presque Isle mill.
OSHA has cited Columbia Forest Products for 15 violations of workplace safety standards and proposed a total of $119,500 in fines, following an investigation after employee Thomas Bray died in a March 22 accident, according to the Bangor Daily News. Bray was caught in the moving parts of a machine that activated while he was performing maintenance, and an OSHA investigation found that the machine had not been disconnected from a power source to prevent an unintended start. OSHA issued the company one citation for that accident with the maximum penalty of $70,000, and 14 other citations for other violations, including defective forklifts, lack of eye flushing stations and electrical hazards, according to the paper.
Columbia Forest Products, the largest hardwood, plywood and hardwood veneer manufacturer in North America, has 15 business days to comply with the citation requirements, meet with OSHA or contest the findings. The company did not respond to the paper's request for comment.
