I'm still in wonderment as to why anyone would ever use tobacco for any reason to begin with. It has been conclusively shown to cause cancer, emphysema, heart disease, atherosclerosis and a whole host of other maladies, so why use tobacco at all to start with? One of the higher costs to health insurance companies is the care required by smokers or tobacco chewers for treatment once the cancer sets in, perhaps as an incentive the insurance companies could offer reduced premiums for non-tobacco users.
